What we read in this article
The eyes are often one of the first areas of the face to show signs of aging. As collagen and skin elasticity gradually decrease, the upper eyelid can become looser and the eyebrow may descend. This can create a tired, heavy or aged appearance, even when the rest of the face remains relatively youthful. Traditionally, significant upper-eyelid laxity and eyebrow ptosis have been treated surgically with procedures such as blepharoplasty or surgical brow lifting. But what about patients who are not ready for surgery? Emerging research suggests that Endolift laser treatment may provide a minimally invasive option for selected patients with upper-eyelid laxity and mild eyebrow descent. A clinical study published in Skin Research and Technology specifically investigated this question—and the results are particularly interesting.

What Is Endolift Eyebrow Lift?

Endolift is a minimally invasive laser treatment designed to deliver controlled laser energy beneath the skin through extremely thin optical fibers. Unlike traditional resurfacing lasers that primarily treat the skin from the outside, Endolift works within the tissue. For the upper face and periorbital region, the objective is not to surgically remove excess skin. Instead, treatment aims to produce tissue tightening and remodeling, potentially improving the appearance of upper-eyelid laxity while subtly changing eyebrow position. This makes the concept fundamentally different from surgical blepharoplasty.

Why Does the Eyebrow and Upper Eyelid Change With Age?

Facial aging is not limited to wrinkles. Changes occur across multiple anatomical layers, including the skin, connective tissues, fat compartments and supporting structures. Around the eyes, these changes can contribute to:
  • Upper-eyelid skin laxity
  • A heavier-looking upper eyelid
  • Lower eyebrow position
  • Loss of definition around the eyes
  • Periorbital wrinkles
  • A tired or aged appearance
Because the skin around the eyes is particularly thin and anatomically delicate, treatment of this area requires careful patient selection and precise technique.

What Does the Scientific Research Show?

A 2023 clinical study titled “Evaluation of eyebrow position and upper eyelid laxity after Endolift laser treatment” investigated Endolift specifically for this purpose. Patients with eyebrow ptosis and upper-eyelid laxity underwent a single Endolift treatment and were evaluated again six months later. Importantly, the researchers didn’t rely only on subjective before-and-after impressions. They used standardized photography, assessment by blinded physicians and digital ImageJ measurements of eyebrow position. This allowed the researchers to objectively evaluate whether eyebrow height actually changed after treatment.

71.5% Improvement in Upper-Eyelid Skin Laxity

One of the most striking findings involved upper-eyelid laxity. The average skin-laxity score decreased from: 3.5 before treatment → 1.0 six months after treatment This represented a reported:

71.5% improvement in skin laxity

The difference was statistically significant. The overall cosmetic-result score also improved from: 3.5 before treatment → 1.2 after treatment representing approximately:

71.1% improvement in the overall cosmetic assessment

These findings suggest that the treatment’s effect extended beyond a subtle change in eyebrow position and included visible improvement in upper-eyelid tightening.

Did Endolift Actually Lift the Eyebrow?

This is where the study becomes especially interesting. Rather than simply stating that patients “looked more lifted,” researchers measured eyebrow height at four different anatomical positions. They evaluated:
  • Eyebrow height from the center of the pupil (CPEBH)
  • Central eyebrow height (CEBH)
  • Lateral eyebrow height (LEBH)
  • Medial eyebrow height (MEBH)
All four measurements increased significantly six months after Endolift treatment.

Objective Eyebrow Height Changes

Center-of-pupil eyebrow height: 16.11 mm → 17.51 mm Increase: 1.40 mm Central eyebrow height: 14.08 mm → 15.57 mm Increase: 1.49 mm Lateral eyebrow height: 16.15 mm → 17.52 mm Increase: 1.37 mm Medial eyebrow height: 19.13 mm → 20.28 mm Increase: 1.15 mm All of these changes were statistically significant. This is important because even a relatively small change measured in millimeters can potentially influence the appearance of the eyebrow and upper eyelid.

A Natural Lift Rather Than an Artificially Elevated Brow

The goal of a good eyebrow treatment should not necessarily be to create the highest possible eyebrow. Facial aesthetics depends on proportion. An excessively elevated lateral eyebrow can create an unnatural or surprised appearance. For appropriate patients, the objective of Endolift is therefore better understood as subtle tissue repositioning and tightening, rather than dramatically changing the natural architecture of the face. The ideal result should make the upper eye appear more open and refreshed while maintaining the patient’s natural expression.

Why Can Endolift Results Continue Developing?

Endolift is not simply an immediate mechanical lifting treatment. Laser energy interacts with the tissue and initiates a remodeling response. For this reason, the final result should not necessarily be judged immediately after treatment. The study assessed its patients at six months, when significant improvements in both upper-eyelid laxity and eyebrow height were documented. This delayed assessment is important because collagen and tissue remodeling take time. Patients considering Endolift should therefore understand that the treatment is better viewed as a progressive rejuvenation procedure rather than an instant surgical-style lift.

Endolift vs Surgical Blepharoplasty

Endolift and blepharoplasty should not be considered identical treatments.

Surgical Blepharoplasty

Blepharoplasty can physically remove excess upper-eyelid skin and, when appropriate, address other anatomical structures. For significant dermatochalasis or substantial excess skin, surgery may remain the more appropriate option.

Endolift

Endolift does not surgically remove a large amount of excess skin. Instead, it may be considered in selected patients seeking:
  • Improvement in mild-to-moderate upper-eyelid laxity
  • Subtle eyebrow elevation
  • Tissue tightening
  • A less invasive approach
  • Less recovery compared with surgery
The correct treatment depends on the patient’s anatomy and expectations.

Endolift vs Botox Eyebrow Lift

Botox can also create a subtle chemical brow lift by selectively relaxing muscles that pull the eyebrow downward. However, Botox and Endolift work through completely different mechanisms. Botox: modifies muscular activity. Endolift: targets tissue tightening and remodeling. In selected patients, these treatments may therefore address different components of upper-face aging. Treatment should be individualized rather than applying the same technique to every patient with a low or heavy eyebrow.

Endolift vs RF and Other Non-Surgical Treatments

Several non-surgical technologies have been investigated for periorbital tightening, including:
  • Radiofrequency
  • Fractional lasers
  • CO₂ laser
  • Er:YAG laser
  • Other energy-based technologies
Each treatment acts differently and has its own advantages, limitations and recovery profile. Ablative laser treatments, for example, can provide significant resurfacing but have also been associated with potential complications such as prolonged erythema, pigmentary changes, infection and scarring. Endolift represents a different approach because it is described as a non-ablative laser technique targeting the tissue internally rather than performing conventional surface ablation.

Is There Downtime After Endolift Eyebrow Treatment?

The study specifically monitored patients for erythema and edema after treatment. The authors reported no side effects among the patients in their final results and noted that the procedure did not require general anesthesia or a conventional surgical recovery period. However, this should not be interpreted to mean that every patient will experience zero swelling, redness or temporary discomfort. The periorbital area is particularly sensitive, and individual responses can vary.

Is Endolift Around the Eyes Safe?

The area around the eye requires considerably more caution than many other areas of the face. Endolift treatment in this region should therefore be approached as an advanced anatomical procedure, not simply another facial laser treatment. Understanding the anatomy, appropriate treatment depth, patient selection and energy delivery is particularly important around the orbital region. It is also essential to recognize the limitations of the available evidence. The study discussed here ultimately evaluated only nine treated patients. Although the results were statistically significant and encouraging, the authors specifically stated that further studies involving larger numbers of patients are needed. Therefore, the evidence should be viewed as promising early clinical data, rather than proof that every patient will achieve the same degree of improvement.

Who May Be a Good Candidate for Endolift Eyebrow Lift?

Endolift may be considered for carefully selected patients concerned about:
  • Mild eyebrow descent
  • Upper-eyelid laxity
  • Early signs of periorbital aging
  • A heavy appearance around the upper eyes
  • Loss of definition around the eyebrow
  • Desire for a non-surgical or minimally invasive treatment
Patients with significant excess eyelid skin, substantial eyebrow ptosis or functional impairment may require surgical assessment instead. This is why consultation and examination are particularly important.

Can Endolift Replace Blepharoplasty?

Not for every patient. The objective should never be to replace surgery simply because a patient prefers a non-surgical procedure. Instead, the question should be: Can Endolift realistically achieve this particular patient’s goal? For mild-to-moderate laxity, a minimally invasive approach may be reasonable. For severe excess skin, significant ptosis or major anatomical changes, surgery may provide a more predictable correction. Good aesthetic medicine depends as much on knowing when not to perform a treatment as knowing how to perform it.

Endolift Eyebrow Lift in Abu Dhabi

For patients considering Endolift in Abu Dhabi, treatment planning should begin with assessment of the entire upper face rather than looking only at the eyelid. The eyebrow position, forehead anatomy, eyelid laxity, skin quality, facial symmetry and muscular activity all contribute to the final appearance. In some patients, the primary concern is skin laxity. In others, eyebrow position is more important. And for some patients, a different treatment—or a combination approach—may produce a better result.

Advanced Endolift Treatment with Dr. Erfan Rahmani

My approach to Endolift treatment in Abu Dhabi focuses on individualized anatomical assessment and natural-looking results. The objective is not simply to “lift” every patient in the same way. It is to determine where the aging process is occurring, which tissue needs to be addressed, and whether Endolift is the appropriate treatment for that anatomy. For suitable patients, Endolift can be considered as part of a personalized strategy for: Upper-eyelid tightening • Eyebrow lifting • Facial tightening • Jawline contouring • Double-chin treatment • Face and neck rejuvenation A consultation is necessary to determine whether Endolift, another non-surgical treatment, or surgical assessment would be the most appropriate option.
